Copy of Government-Issued Photo ID

Assets:

Checking, Savings, Mutual Fund, Investment Account, etc. in your name for the last 2 months

Please note bank statements should include all numbered pages, even blank pages.

If you own other properties:

Please provide a mortgage statement and tax returns

For W-2 Wage Earners (if you are employed):

Paystubs for the last 30 days

W-2’s for last 2 years

Federal Tax Returns Form 1040 (with all schedules) for last 2 years

For Self-Employed & Commissioned:

Federal Tax Returns Form 1040 (with all schedules) for last 2 years

For “Other Income”:

Disability Award Letter

Social Security Award Letter (EVERY YEAR A NEW LETTER IS GIVEN)

Retirement Benefits Award Letter

For Divorced Applicants:

Divorce decree and any related information, which pertains to alimony, child support or separate maintenance.

Veteran (VA) Loans:

VA certificate (DD214 Discharge Letter)

VA Disability Letter (if applicable)
